WAIVER AND RELEASE

In consideration of permission granted to me/my child and I use Barry’s boxing facility location at 2664 South Highland, Las Vegas Nevada 89109 and to participate in training and boxing events, both competitive and non-competitive.

I/We the undersigned do hereby release and discharge Barry’s Boxing Center. Its agents, employees, from all claims, demands, actions, judgments, and executions which the undersigned’s heirs, executors, administrators, or assigns may have, or claim to have against Barry’s Boxing, it’s successors or assigns, for all personal injuries, known, and injuries to property, real or personal. Caused by, or arising out of, the above described sport activities.

I/We, the undersigned, fully understand that this sport activity has inherent risk involved, but fully waive rights, claim, cause of action, etc., as heretofore enumerated and do(does) hereby assume risk.

I/We, the undersigned, have read this Release/Waiver and understand all its terms and conditions and I/We execute it voluntarily and with full knowledge of its significance.

The purpose of our website is to inform people about our program and showcase the achievements of our members. From time to time, we may feature photos and/or writings from members and affiliates on our website and/or social media accounts. As this site can be accessed worldwide, we have provided a permission form below. For security reasons, only first names will be used. Please sign the form below.

I grant permission for my photo and/or writing to be included on the website for Barry's Boxing Center or any of our social media accounts.

I understand that this website is publicly accessible on the World Wide Web (WWW) and can be viewed by people with Internet access worldwide.

For security reasons, only my first name will be used.

BUSINESS HOURS

REQUIREMENTS

Due to the COVID-19 we have been forced to make some changes that WILL be strictly adhered to. If you for any reason you cannot I am sorry to say you cannot work out here.

READ THE FOLLOWING BEFORE YOU JOIN

1)      As per the sign there is no waiting room. It is a drop off and pick-up area ONLY!

2)      NO NON-MEMBERS ARE NOT ALLOWED TO ENTER THE GYM!

3)      YOUR GYM DUES MUST BE PAID BEFORE YOU ENTER THE GTM! NO EXCEPTIONS!

4)      Any and all items left in the gym or the lost and found or in lockers will be thrown away at the end of each month. That would include shoes, shirts boxing gear jewelry, etc. I will NO longer store you belongings for any period of time.

5)      Any and all equipment not put back cleaned and where you got it from will be taken away and NOT replaced.

6)      Clean the equipment AFTER you use it! There is cleaning solution available to you.

7)      BRING A TOWEL DAILY, it’s your sweat wipe it up

8)      NO FOOD OR DRINKS ALLOW IN GYM EXCEPT WATER

9)      WASH YOUR HAND………..ESPECIALY AFTER USING THE REST ROOM

10)  You Must be registered with USA Boxing to work out here, (per the NSAC)

11)  Drinks are not free, if you want one pay for it, I have to pay for them

12)  ALL WORKOUTS ARE 90 MIN TO 2 HOURS ONLY! WE ARE NOT CHILD CARE

13)  Gym hours for amateurs start at 4pm, a condition class is M-F at 4:30 only, the gym is also available on Saturdays from 11 to 3 pm only.

14)  After the first month you are required to buy boxing shoes and your own equipment. We do not care where you buy it just make sure the tag says USA approved.

15)  NO horse playing, if you do not participate you will be asked to leave

16)  If you are sick STAY HOME!

17)  If you show signs of COVID-19 SEEK MEDICAL ATTENNTION

18)  WORK OUT OR GET OUT, DO NOT TAKE TIME FROM OTHERES

19)  We close at 8 pm not any later.

20)  We reserve the right to refuse service and adjust hours when necessary

ALL OF THE ABOVE MUST BE ADHEARED TO OR YOU WILL BE ASKED TO LEAVE. THERE IS NO ARGUEING OR DEBATING WE ALSO HAVE TO MEET MANDATES; THANK FOR YOUR COOPERATION IN ADVANCE HOPE TO SEE YOU SOON!

It is the Policy of Barry's Boxing Center to have ZERO TOLERANCE CONCERNING ANY AND ALL GANG ISSUES!

Graffiti, Intimidation Tactics, Destruction of property, (Gangster type) dress attire and gangster rap music are all gang issues!

If you are affiliated with any gang or are a wannabe, PLEASE DO NOT COME HERE!

We do not tolerate any gang related flying of Colors, or Dress. We will not allow any gangster rap music, played or listened to at any time. There are NO ELECTRONIC DEVICES ALLOWED IN GYM.

THERE WILL NOT BE ANY ILLEGAL DRUGS OR WEAPONS ALLOWED ON THE PREMISES OR IN YOUR POSSESSION AT ANY TIME.

IF THESE RESTRICTIONS ARE TOO DIFFICULT FOR YOU TO ADHERE TO, THEN PLEASE CLOSE THE DOOR ON YOUR WAY OUT!

Any gang or other Criminal Activity discovered will be prosecuted to the fullest extent of the LAW and result in immediate expulsion from Barry's Boxing Center!
